Instructions:
You may wish to copy each portion of the appliction, (there are three sections/pictures) paste it into a Word or other type document and resize it before printing.
- Complete all sections and information.
- Include all requested attachments.
- Incomplete applications or those with missing information will not be considered.
- Return two copies of the entire application and all attachments in a sealed envelop clearly marked “Miller Scholarship Application, Attn: Trust Department” to any office of The Honesdale National Bank no later than 3PM EDT May 15, 2008.
